International Shipping Export Documents
Ocean Star International offers an online system to book your international shipment which you can access here. If you’re ready to book your shipment, and rather access the international shipping export documents manually, you can simply download the documents below. Please make sure to fill them out in their entirety and email, scan or fax them to the relocation expert you’ve been speaking with. Please note that all documents must be filled out in order for to schedule the pick up of your goods. The forms below are in PDF. and Word format so you can print them out fill them out and send them back once completed. Step 1. Shipping Information Form – Download
Please make sure to have your United States and destination address included on this form. If you do not have an address or your service is door to port we will still require an address. If you do not have an address you can use a friend or family members address for now. Please also provide us with any contact phone numbers, emails, or fax numbers we can use to reach you at all times. Please also remember to provide us with a passport # on this form. The more information you provide us the better.
Step 2. Owner Packed Inventory list – Download
Please complete the Inventory form, with a description of the items in each of your boxes, and place a value next to each box. Example; Box 1 Clothes, Books = $500, Box 2 DVDs, linens $300, etc., etc. You do not have to provide us with a detailed inventory list of every single item. This form is so we can get a general idea of what’s in each box. We will not need receipts. Please make sure to include the goods declared value.
Step 3. Platinum Plus Insurance Request- Download
**This form is only for professionally packed customers. If you’re self packing please fill out the insurance forms below**
Platinum Plus insurance is for professionally packed customers only. Per the insurance companies stipulations, you must insure the total value of your shipment. If you insure your shipment for less than the value disclosed on your inventory list you will only be reimbursed a pro-rated portion of your coverage. Please try and be as accurate as possible to avoid any coverage issues should a claim be filed.
Named Perils Insurance Request Form **If you’re self packing your goods please fill out the Named Perils Insurance form**
Per the insurance companies stipulations, you must insure the total value of your shipment. Step 4. OSI CC form – Download
The credit card form is for the initial deposit. Please keep in mind you can also pay by check, wire transfer, or money order. If you do not have the means to sign the bottom of the form, and scan back to us, we will accept your name and date typed. If you have any questions regarding the payment please let us know.
Step 5. TERM & CONDITIONS OF SERVICE – Download
